Person-centred thinking tools and skills provide a practical framework you can use day-to-day to help people to have more choice and control in their life.

What is person-centred thinking?

It is a set of seven practical skills and tools used to personalise health, education and social care services. They can be used with people you support, patients or children, and also with colleagues, teams and in organisations. They help us to deliver personalised support by guiding us through the discovery of what is important to a person; how to best support them; the way they communicate and make decisions; and how to move to outcomes and action.

Each person-centred thinking tool provides the basis for actions and further information about what is important to people and how they want to be supported.

Who is this for?

Person-centred thinking skills and tools are the way to deliver personalisation on a day-to-day basis. If you support someone in your role or you are a manager, professional, parent or carer, then this course is for you. It is also ideal if you need an overview of the core person-centred thinking tools.

  • How to separate what is important to someone from what is important for them and how they want to be supported.
  • How to capture what people appreciate about the person.
  • How to record this information into a one-page profile.
  • Discovering what is working and not working in a situation from different perspectives leading to action plans that make a positive difference in people’s lives.
  • Recording how someone communicates using a communication chart.
  • How to use the Decision Making Agreement.
  • The many different ways of recording one-page profiles.
  • How to decide which person-centred thinking tool to use to develop a one-page profile into a person-centred description (or support plan).
  • How to identify what core responsibilities are and where you can use creativity and judgment.
  • How to match people based on skills, qualities and interests.
  • How to mindfully record learning by using Learning Logs and 4 plus 1
  • What the benefits of using one-page profiles with staff are; for example as part of person-centred team plans.
  • How using person-centred thinking and one-page profiles can help to deliver personalised services.
