Six innovations in social care

In February John Bolton brought together a group of innovators in social care. One of the outcomes was this summary of 6 innovations. They include Wellbeing Teams and Community Circles.
Each organisation has developed a one-page summary of their innovation, which covers:
- the problem they are aiming to solve
- the solution they have created
- their evidence base and expected impact
- the stage they are at currently
- what else is needed to support it
- and what would stop the innovation from happening successfully
This document is useful for anyone who is interested in new models of care and support, and would like to find out about different innovations that are being developed across the UK at the moment.
