Working Together For Change

Working Together For Change (WTFC) is a way to collate information from the individual support plans and person-centred reviews of people your organisation supports, and analyse it to create a bigger picture of what is working and not working. This in turn creates a data-driven, person-led, strategically-driven vision for the future. We usually deliver WTFC with your team over two days, but this can be tailored to suit your needs or integrated into a wider programme of HSA support.

Switched On Support

‘Switched On Support’ is a range of programmes designed to support organisations to use connected care, telecare and other assistive technologies in a way that is effective, person-centred and focused on helping people to achieve their desired outcomes.

Developing a One-Page Strategy

To make change happen, it is essential to easily be able to communicate your vision to your stakeholders.

A One Page Strategy captures what success means in your organisation, what you’ll do to deliver it and how you’ll know how well you’re doing. We can facilitate a session to co-produce a One Page Strategy with your organisation in just one day, or incorporate it as part of a wider programme of work.

 

Evaluate your service with 'Progress for Providers'

Progress for Providers is a series of assessment tools, designed to enable organisations to know how well they are doing in providing truly person-centred support and care. You can undertake Progress for Providers yourself using our self-assessment tool, or we can facilitate the process alongside your team.

 

Transforming Teams in Health and Social Care

This Transforming Teams programmes for Health and Social Care assists you to identify the changes that you want to make for the people you support or your patients, with the support of your staff teams, in order to give them a more personalised experience with increased choice and control.

We then help you to achieve the change you want to see through the creation of a structured plan, as well as supporting you throughout the practical steps towards implementation and evaluation.

 

Transforming Hospices

The Transforming Hospices programme works with hospice teams to define what a better end of life experience would look like, and then to create a plan to make it happen, together.
